Care Support Coordinator

Job Overview

Lyon-Martin Health Services provides excellent health care to heterosexual women, bisexual women, lesbians and transgender people in a safe and compassionate environment, with sensitivity to sexual orientation and gender identity; all services are regardless of ability to pay. The Care Support Coordinator (CSC) works closely with both our medical and mental health teams to provide trauma-informed medical case management to patients with complex psycho-social barriers to care, using a social justice framework. An ideal candidate is highly-motivated, organized, flexible and adaptable; capable of working in a fast-paced environment independently, as well as on a team.

Key Responsibilities

  • Conduct appointments with patients to provide medical case management, accompaniments to specialty care appointments, internal/external resource referrals, and assistance with filling out medical forms.
  • Be on-call during clinic hours (4 shifts a week) for medical team patients who need suicidal ideation assessments, de-escalation, crisis intervention and emotional support.
  • Assist patients in navigating complex insurance issues, and advocate effectively and professionally with insurance representatives.
  • Assist with administration tasks related to the Mental Health Team.
  • As is true of all Lyon-Martin Health Services staff and volunteers, the Care Support Coordinator is also responsible to uphold and imbue in others a commitment to addressing racism, and gender and sexual discrimination within the organization and our clinical practice.

Education and Knowledge, Skills and Abilities

  • Ability to de-escalate angry or upset patients and provide emotional crisis support.
  • Familiarity with trauma-informed care, harm-reduction models, and social justice/anti-oppression concepts.
  • Strong relationship-building and conflict-resolution skills.
  • Ability to manage a variety of diverse tasks and responsibilities, meet deadlines and prioritize effectively.
  • Excellent communication skills and ability to navigate “difficult conversations” with patients.
  • Demonstrate good judgment in unusual and emergent situations.

Compensation: $40,000.00-$42,640.00
