HealthRIGHT 360 is currently looking to fill a Tactical Nurse position opening at our Lyon-Martin Health Services program in San Francisco, CA! The ideal candidate has 2 years of direct patient care experience working in a community health center or primary care setting, experience working with diverse populations, and has successfully worked with issues of substance abuse, mental health, and medical complexity. In this position you will gain experience in providing transgender health care as a part of a health center with a national reputation in transgender health. Additionally, this position offers the ability to see patients independently and utilize the full scope of your RN license in an integrated behavioral health/primary care model! Sound interesting? PROGRAM DESCRIPTION:

Lyon-Martin Health Services provides excellent health care to women, lesbians and transgender people in a safe and compassionate environment, with sensitivity to sexual orientation and gender identity; all services are regardless of ability to pay

The health center was founded in response to the lack of understanding for how to provide evidence-based health care to lesbians. Today, the health center responds to gaps in care for all women and transgender people, providing primary care and integrated behavioral health services to create a medical “home” for our patients. Lyon-Martin stands at the forefront of lesbian and transgender health care issues, and has assumed a position of national prominence in the area of LGBT health services.

In March of 2015, we underwent a strategic merger with HealthRIGHT 360. While preserving each organization’s unique specializations and orientation to care, the strategic alliance bolsters both groups in the changing landscape of healthcare under the full implementation of the Affordable Care Act.

Key Responsibilities

  • Provide nursing care, within the scope of license and agency credentials, to Lyon-Martin patients within a Patient Centered Medical Home delivery model, utilizing motivational interviewing skills and encouraging patient activation and self-management.
  • Facilitate patient care team functioning and efficiency by taking responsibility for additional patient assessments or medical assistant functions (e.g., phlebotomy, rooming, vitals, etc.) to ensure the completion of patient sessions in a timely and thorough manner.
  • Assess and provide nursing advice to patients who walk-in with medical complaints or who call-in with medical concerns seeking advice.
  • As a part of the Care Support Team, prioritize and take responsibility for patients with complex medical issues, follow up with all patients regarding recent emergency room visits and inpatient stays, and be an active team member through case-conferencing and other collaborative work.
  • Work collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes.
  • Provide clinical leadership to support staff, including medical assistants, patient care coordinators, front office staff and receptionists and participate in training them to improve their skills.
  • Actively participate in Quality Improvement activities and may act as change champion or lead change processes.
  • In conjunction with Operations Director and Medical Director, assure organizational readiness for accreditation surveys and ongoing monitoring and reporting of conformance to quality within the clinic.
  • Demonstrate commitment to mission of the organization, queer health, racial justice and community-oriented primary care.

Education and Knowledge, Skills and Abilities

  • Possession of a valid RN license
  • Bilingual/fluent in English/Spanish
  • Two years of direct patient care experience in a primary care setting or a community health center, preferred
  • LGBT cultural competency and ability to work with diverse populations
  • Experience working successfully with issues of substance abuse, mental health and medical complexity
  • Knowledge of co-occurring disorders and trauma-informed care
  • Excellent verbal, written and interpersonal skills
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility
  • Strong initiative and enthusiasm and willingness to pitch in whenever needed
  • Experience working with electronic health records (E-clinical works, preferred)
