Are you passionate about providing high quality primary care in an integrated team setting?   Do you have a commitment to the Lyon-Martin Health Services mission? Are you bilingual in Spanish and English?  Are you passionate about helping people make and sustain change in their lives?  If so, HealthRIGHT 360 is currently looking to fill a Nurse Practitioner/Physician Assistant position at our Lyon Martin Health Services program in San Francisco, CA!  The ideal candidate can work with a diverse population, has LGBT cultural competency, and has experience working successfully with issues of substance abuse, mental health, and medical complexity.  Additionally, we’re looking for someone that has knowledge of co-occurring disorders and trauma-informed care.  In this position you will gain experience in providing transgender health care as a part of a health center with a national reputation in transgender health.  You’ll also have the ability to see patients independently and utilize the full scope of your license in an integrated behavioral health/primary care model.  Sound interesting?  Below you will find a more detailed description of some of the duties and requirements for this position.

Lyon Martin was founded in response to the lack of understanding for how to provide evidence-based health care to lesbians. Today, the health center responds to gaps in care for all women and transgender people,  providing  primary care  and integrated behavioral health services  to create a medical “home” for our patients.  Lyon Martin stands at the forefront of lesbian and transgender health care issues, and has assumed a position of national prominence in the area of LGBT health services.  We have a diverse patient population and a fun and friendly work environment.  The ideal candidate understands that offering patients a professional setting where they are treated with dignity, respect and compassion isn’t just lip service; it is the way and reason that we run our organization.

In March of 2015, we underwent a strategic merger with HealthRIGHT 360. While preserving each organization’s unique specializations and orientation to care, the strategic alliance bolsters both groups in the changing landscape of healthcare under the full implementation of the Affordable Care Act.

Key Responsibilities

  • Provide primary health care to adults and adolescents under the care of the Medical Program. This includes management of common acute and chronic illness as well as all stages of HIV Disease and chemical dependency.
  • Collect a comprehensive history on all continuity patients.
  • Perform ongoing assessment of chronic disease conditions.
  • Formulate diagnoses based on interpretation of health history, physical exam and diagnostic tests.
  • Identify patients at risk for specific health problems.
  • Utilize all the above information in formulating and documenting health care plans which include treatment, health maintenance, education, counseling, and follow-up.
  • Maintain proper standards of documentation; work with the Medical Director to ensure that medical records are complete, and that appropriate charting and documentation is performed.
  • Furnish or transmit medication orders as designated by licensure and according to clinic procedure.
  • Identify staff and volunteer training and professional development needs.
  • Participate on various clinic teams and committees working with other clinic staff to strengthen the clinic. Continually provide ongoing assessment of the medical as well as psychosocial needs of the clinic’s patient population. Participate in the regular evaluation of existing programs and in discussion of potential new programs. Assist in the implementation of new programs as needed.
  • Attend staff meetings, retreats, training and other Clinic and community meetings as required.

Education and Knowledge, Skills and Abilities

  • Possession of a current, valid California Registered Nurse License; AND
  • Possession of a California Nurse Practitioner Certificate; OR
  • Possession of a current, valid California License to practice as a Physician Assistant issued by the California Board of Medical Quality Assurance
  • Experience in chemical dependency treatment and recovery
  • BLS/ACLS certification
  • Clinical experience with and commitment to working with diverse client populations including the LGBTQ, homeless, persons with substance abuse and mental health issues.
